Once you've followed the link for the Challenge Grant or the Healthy Futures Grant:
You'll need to create an account. If you work for a larger organization like the Health Department or the Wilkes County School System, likely there will already be an account created for you. Call (336) 838-1949 to be added to your organization's account.
Click "Apply" in the lefthand sidebar and select which grant you're interested in.
If you wish to make application to the Healthy Futures program, you must complete the Letter of Inquiry first. Once it is determined whether your program matches with our guidelines and interests, you will be given access to the full application.
Healthy Futures grants are awarded on an open timeline. To receive a funding decision at our August 2017 board meeting, you must complete and submit your grant by Monday, June 21st.

The Health Foundation, Inc. supports programs and services that improve the health and well-being of the citizens of our area. Support is generally limited to: 1) non-profit organizations with tax exempt status as described in Section 501(c)(3) of the Internal Revenue Code or corporations with tax exempt status as described under Section 170 (c)(2) of the Internal Revenue Code; or 2) scholarships to individuals pursuing an education in health related fields.
Organizations and activities associated with elementary and secondary schools (either public or private) such as PTA’s, PTO’s, booster clubs, carnivals, etc.;
Civic clubs, social or fraternal organizations;
Individuals (except for scholarships);
Annual operating funds;
Athletic programs, events, facilities and organizing committees;
Tables, galas, tickets to "benefits" and other fund raisers for annual operating funds or special projects;
Organizations that require membership in a particular political party or religion in order to receive services;
